PRINT THIS! Blood Pressure Diary

Print in full color print in black and white

If you are at risk for hypertension, start taking regular readings at the same time of day. Print this chart and use it to keep track of your systolic and diastolic numbers. Save this chart in a place you can easily access and be sure to bring it to your next doctor visit. If you have hypertension, begin taking readings twice a day and record your numbers in an easy and reliable way that works best for you. If you’re worried you might lose this chart, you can also keep a dedicated journal, spreadsheet, or note in your phone, or download a blood pressure tracker app, like Smart BP or Welltory. You may be able to get an at-home monitor through your health insurance, or use a public monitor at your local pharmacy.
